Complete set
Case materials, recordings, reports, medical documents and information about children are agreed separately after the legal basis is checked.
Authority
A representative confirms their authority to act for the client and submit materials.

Do not attach health data or a child's documents through the initial form. The method for submitting the complete set is agreed separately.
